All new clients please complete the Disclosure, Privacy Rights, Payment Agreement and
appropriate Family System Intake document.

You do not need to provide me with a signed release unless previously discussed.

Couples can print two copies of the intake document for couples
and each complete one.

Kids can complete the appropriate portions of the family system
intake for kids and a parent can then add their thoughts to the same
document.

New clients can download, print and fill out these pdf forms at their convenience.
